Hey, everybody, it’s Dr. O here from RevitalizeMaui. Today we’re going to be talking about our ReFirme treatment, which is geared at targeting loose and sagging skin. We’re going to review how it works, who it’s for and what you can expect from getting this treatment. Now, be sure to keep watching because at the end, we’re going to have some great information regarding all those things and maybe even get you a little teaser on how you can save on getting this done.

This is really for anyone who’s got issues with sagging skin, okay? And the reality is, as we age, we lose collagen, elastin and hydration in our skin. The combination of these effects turns into issues with loose or sagging skin. It can also be compounded with exposure to the sun, cigarette smoking and genetics. You can actually have issues where not only is the skin sagging, but you can have deeper wrinkles, you can have moderate to severe wrinkles. You can lose that shine or that glow in your skin. The skin can take on a sort of gray doll appearance.

Now, when I talk to my patients about what we do to correct some of these issues, of course, we focus on the easy things first. Yeah, sunscreen, sunscreen, sunscreen, some proper use of products, hydration, balancing hormones, things like that. But after we’ve taken care of the basics, really, now we’re thinking about what we can do to turn the clock back to improve the quality of your skin.

Ultimately, we want to think about things we can do to address some of these concerns. And in order to do that, I like to look at what’s happening anatomically to decide on the correct treatment. Let’s just talk about what’s actually happening when you look at the skin. There are several different layers of the epidermis. And what we have to look at is what’s happening at the top layer of skin and what’s happening in the middle layer of skin.

Reduce Saggy Skin

So obviously, things that will resurface your skin’s epidermal area-the epidermal layer, are going to remove those fine wrinkles and maybe even some medium wrinkles. There are many things you can do, such as Laser treatments, chemical peels, IPL, CO2 lasers. However, what we’re really talking about now is what’s happening on the deeper layers of skin where the collagen and elastin are missing.

So what we’ve done with this treatment is combine a really great industry-standard treatment with the Erbium laser for targeting the top layers of skin in combination with something to bring more collagen to the deeper layers of skin. When you combine these two things together, we really have a profound impact on skin quality and the overall tightness of the skin.

So this is why we call this one the ReFirme treatment. Okay, now it’s time to get into how it works. So I think the best place to start is with the deeper layer of the skin. Most of you have heard of microneedling, right?

I used to be a huge fan of micro-needling with PRP, for example. What that’s really doing is using mechanical trauma such as micro-trauma to cause the skin to react by building collagen. That’s the basics really to all of these treatments and how they work. They cause small amounts of trauma to the skin, each at different layers to cause your skin to build more collagen.

Now, what we have here is this is a radio frequency micro-needling platform. On this device, there are gold pins; these deliver energy at different layers as we set into the skin.

So when we are doing, for example, the cheek area, we’re going to use about the specific level of power at 1.2 to 1.5 mm into the skin. It gives a mechanical trauma that you get from micro-needling. However, in addition, it causes a thermal injury that shrinks the dermal tissues and causes an even greater release of collagen over time. By focusing on those deeper layers of tissue. Now, we combine that in the same treatment with one of my favorite treatments for skin resurfacing, the Erbium laser.

The Erbium laser targets water as the chromophore. It uses a wavelength of 29, 40 nm. This is very important because this wavelength does not affect melanin or hemoglobin in the skin. So it’s not really looking for hair pigment, but really focusing on really vaporizing the tissue.

Okay, so when we’re using the Erbium to vaporize a layer of about 100 to 130 microns of tissue. The focus is on the top layer of skin so that you remove that old skin and your body is forced to regenerate a new fresh layer of skin. Now, how we compare this to something like CO2 is this is a much more gentle and lower downtime treatment. You’re not going to be swollen and peeling for three to four weeks like you would in the old days of the deep CO2. This is a gentler way to do this.

Now, because of that, we’re going to do both of these treatments twice, about four to six weeks apart. What we find is when we do it this way, we have excellent results. We have low downtime, and it’s controllable. We’re going to tailor the treatment to what your needs are. If you need more, we’ll start off a little bit higher energy levels, repetition levels, more passes, and we’ll be able to really deliver a fabulous result for you.

The other thing I want to point out with this treatment is, there are actually two parts to it.

What we do first is Erbium treatment using a six-millimeter spot size. And what that means is we are totally removing the skin in little six-millimeter chunks. So it takes a fair amount of work, but we blast off all that skin, revealing really smooth skin below. We then follow that with a fractional lens.

A fractional treatment is one where it’s not removing the full layer of skin; it’s actually targeting deeper into skin layers, small columns of energy, and the benefit there again is lower downtime, but we can actually penetrate a little bit deeper without having significant side effects. We combine those two, so this treatment, you actually do two separate laser treatments and then we follow it with the Pixel8.

Okay, so here we’re doing three different treatments combined in one, and what we find is when we do it this way, there’s a synergistic response between these three different layers and three modalities and the end result is absorbed, resulting in lower downtime and ultimately greater satisfaction.

Lip Augmentation on Maui

Consultation:

Dr. Okwuje will meet with you to determine your goals. His treatment plan and selection of dermal fillers will depend on how much you want to plump your lips. Do you want to rejuvenate your lips or augment them?

Building a Personalized Treatment Plan:

Dr. Okwuje does not believe in a “one size fits all” aesthetic plan. There is a great deal of variation between lip shapes and sizes, and each patient is unique. He will devise a treatment and enhancement plan that is as unique as you are. Whenever possible, though, Dr. Okwuje works within the existing anatomy of the lip to preserve its structure for a natural and flawless result.

Approach:

There is a difference between rejuvenating or restoring the lip and adding volume or augmenting the lip. For patients desiring to restore their lips to their youthful contours, Dr. Okwuje considers the underlying structure.

In mature patients, the maxillary bone (the bone just under the under lip) begins to erode, causing the lip to turn inward. For these patients, Dr. Okwuje’s most important consideration is averting the lip by restoring it to its original construct without adding too much mass and volume.

Dr. Okwuje may also eliminate those pesky smoker’s lines, by eliminating those tiny lines that surround the lips and age the face. Your lips will look smooth and refreshed, and your lipstick will no longer bleed around your mouth.

Comfort:

Signs Of A Bad Botox Injection

Here Are the Signs of a Bad Botox Treatment:

FROZEN Forehead

Too much neuromodulator is injected in frontalis muscle – unless, of course, this is what you wanted, all shiny and smooth. We prefer a little movement to look natural and soft.

HEAVY Brow

Some people, especially those with naturally low brows and small foreheads, are more prone to a dropped brow. Can be treated by an expert injector but will resolve in time.

SPOCK Brow

Well… you will simply observe out of control brows arching way high. Luckily this can be corrected easily at your follow up session.

Happens with many treatments around the lips – especially when administering TOO MUCH Botox in the area. This issue can lead to difficulties talking, drinking, smiling, drinking with a straw…yes, we are serious.

Sadly there is more. But the best news is most of these issues wear off within weeks/months without long term issues.

So you have decided that you are interested in possibly getting treatment with dermal fillers, but having trouble figuring out what types of fillers are best for your aesthetic goals? When you are considering your budget you may also be wondering, “Well, how much filler am I going to need to get the results that I am looking for?”

There are several things to take into account when determining the how much filler that you need. One would be the amount of enhancement you are looking for – are you wanting a slight, moderate, or major change? The other would be age. Starting in our mid 30’s we begin to lose approximately 1cc of volume in the facial area per year. Because of this, most patients require 1 syringe of filler per decade of life.

So, for a full correction at 40 years old, you would need about 4 syringes. 50 years old? You’ll need about 5 syringes of filler, and so forth. In combination with age, lifestyle choices, such as smoking and sun exposure, can also affect how much filler you might need.

How Much Dermal Filler is In 1 Syringe?

The majority of dermal fillers are made from Hyaluronic Acid and contain 1cc per syringe. Keep in mind that this means that there is only 1/5 of a teaspoon of filler in that 1cc syringe! This means that it takes FIVE full syringes to make a up a single teaspoon of filler.

How Much Dermal Filler is In 1 Syringe?

It is important to remember how much filler is actually in a syringe when thinking about how much you will need to treat the areas you are concerned with so that you are not disappointed with the outcome of your treatment. For most people, treatment with 1 syringe will be a subtle enhancement, which can be great for younger patients looking to maintain their appearance or those who do not want to see a lot of change.

There are some fillers, like Radiesse, that have 1.5cc of product per syringe. Radiesse is made from another naturally occurring substance in the body, Calcium Hydroxylapatite, and can be a great option for treating certain areas, but it is not an ideal option for every treatment area. So How Much Filler Will it Take?

Slight Enhancement

A slight enhancement would be something like filling in a few fine lines, smoothing or a slight plump in the lips, or having a small amount of hollowness filled in. Usually, a slight enhancement would be focusing on one area to improve upon, without a drastic result or significant change to your appearance, and these treatments only require a small to moderate amount of dermal filler.

However, depending on things like the area being treated and your age, how much filler you need for this type of enhancement can still vary from approximately 0.5cc – 3cc. For example, a patient in their 30s wanting a slight enhancement in the cheeks to look more like the did in their 20s may only need 1cc to see those results, where someone in their 50s or 60s looking for similar results would need at least 3cc.

Moderate-Major Enhancement

Upper Face
The areas considered to be the “upper face” include the forehead and glabella. The glabella is the area between the brows that many people refer to as their scowl lines or “11s.” Once wrinkles have become etched deeper into the skin over time, becoming what we call “static wrinkles,” treatment with neuromodulators like Botox is not enough to completely eradicate them.

First, the area would still need to be treated with neuromodulators, and once the toxin is in full effect small amounts of dermal filler are used to pop out the fine lines that remain. The amount of how much filler usually needed to treat the upper face is 1-2cc.

Mid-Face
When we reach middle age, (gasp!!!), areas in our mid face begin to lose the volume they once had and become flattened. The areas included in the mid-face are the temples, the under eye area (tear trough), the cheeks and nose. Depending on your age and natural deficits, how much filler needed to treat these areas can require a considerable amount.

The tear trough typically requires 0.5-1cc per side, more if you have very large bags. Some people may just need 0.5cc per side in the temples, while others can need 2cc per side. The same goes for the cheeks, usually ranging from 1-3cc per side (yes this means you could need up to a total of 6cc just to lift your flattened cheeks).

If you are looking for a total Mid Face Liquid Lift with full correction, you can expect to need a minimum of 4cc of filler.

Lower Face
The area referred to as the “lower face” includes the marionette lines from the sides of the mouth to the chin, the nasolabial folds from the sides of the nose to the corners of the mouth, the oral commissures at the corners of the mouth, the lip region (which includes lip lines, the lip border and any plumping of the lips), and lastly the jawline.

The nasolabial folds are an area that tends to really bother patients as they continue to get deeper over time with the cheeks dropping and flattening from loss of volume. Replacing this volume in the cheeks helps to lessen the depth of the nasolabial folds, but these lines may still need to be filled in after with 0.5cc to 2cc of filler per side for a nice correction.

Marionette lines typically require 0.5cc – 1cc per side. In order to reverse the turning down of the oral commissures in the corners of the mouth, how much filler needed could easily be 0.25cc – 0.5cc in just this area alone.

Many patients want lip injections for many different reasons, however, it is very common to misunderstand how much filler is required to reach their desired results. The lip lines around the mouth, which are often referred to as “smokers lines” can be an issue even if you never smoked due to loss of volume in the lips, and they may take as much as 1cc – 2cc to correct on their own. The deeper the lines, the more you will need to correct them.

Enhancing the border of the lips can take 0.5cc to 1cc, and the actual lips themselves can need 1cc – 2cc. This means that for a for a moderate – major enhancement, the entire lip area could begin with needing 3cc of filler to correct fully.

Now, that being said not everyone will require that much filler, but this is where things like being a chronic smoker, your age and genetics will come into play. How much filler needed for the jawline could be anywhere from 1cc -3cc, depending on your desired outcome.

I Don’t Want to Look Weird – What About Overfilling?

After seeing how much filler that you might need for a full or partial liquid facelift, you are probably thinking that you would look very strange or “plastic” with that much product in your face. Unfortunately, there have been many celebrities in popular culture able to afford large amounts of dermal fillers who have gone beyond what is needed for a full correction.

This is what we call overfilling, and it results in people looking unlike themselves with possibly bloated or unmovable features. While this “look” has become synonymous with fillers due to the attention that famous people can get for looking this way, it is not a cheap, easy or desirable look when it comes to aesthetic treatment. Dermal fillers were designed to lessen the signs of aging and enhance your appearance, not change it completely.

When it comes to dermal filler injections, more does not always mean better. It is important that your physician match the correct amount of filler to how much filler you need for your desired results. It is always better to place less first and then add more later if needed. This ensures that any changes to your appearance are gradual and natural, and prevents any issues with overfilling.

You should find an injector who has a style that you are comfortable with. If you want to avoid looking overdone or strange, it is best to find one who emphasizes a more natural result, which usually includes the preservation of some lines and imperfections. Because fillers can be almost addictive for some people, it is also best to have a physician who cares more about your end result than lining their pockets, so that they can give you the best advice for when, where and how much filler to inject for a nice result.

You may have also heard horror stories of situations where people have ended up looking strange or “misshapen,” even without a lot of filler being used. This is still possible, and is most often the result of dermal fillers being placed incorrectly. This is why it is also important to find a physician who is experienced with the science and artistry of dermal filler application, and can determine how much filler you need.

Dermal Filler Result Chart

The results from filler procedures tend to follow an inverted J curve. This means that you will look better when the right amount is put in, but then there will come a point where adding more will start to make you look worse. This usually happens when you go beyond simply improving on your appearance to changing it altogether.

At times you may want to make changes to your natural appearance with fillers. For example, someone who has always been self-conscious about having small lips may want to increase their size. However, if you have smaller lips to begin with then there is still a limit to the amount of volume you can add without making them look unnatural or overfilled.

Common Issues From Overfilling

Puffy/Pillow Face
This look is a result of overfilling in both the cheeks and under-eye area. This is seen fairly often, as many Hollywood celebrities have succumbed to this syndrome. Having fuller cheeks is typically associated with youthfulness, however, when you overdo it in someone who is older it results in overly puffy cheeks that look unnatural, out of proportion, and inappropriate for their age.

This can also happen in younger people who are in pursuit of “apple cheeks.” When you overfill the cheeks and then tear trough area under the eyes, it obliterates the natural lid-cheek junction. This causes the entire mid-face to become one continuous bulge, like a pillow, making the eyes appear deeper and smaller.

This problem does not just affect women, and it actually can affect men even more. When you overfill the cheeks in men it results in their face becoming more feminine and can look strange, bloated, and less masculine. This is why you must know how much filler you need for your specific situation.

Maleficent Cheekbones
Having full cheeks can make us look more youthful, while strong cheekbones can go even further to contribute to the “perfect look.” Facial contouring with makeup has become all the rage to help emphasize this look, and when done correctly dermal fillers can go even further to shape our cheeks to make them more defined and youthful.

If they are done incorrectly, either due to how much filler used or placement, you may risk looking like Angelina Jolie’s character in the movie Maleficent with overly protruding cheekbones, creating a very “hard”, unnatural look. Your doctor should work with the natural shape of your face to determine where the best placement and how much filler needed to avoid this result.

Duck Lips
Possibly one of the most common offenders on this list is the dreaded duck lips. This is one of the worst because it is seen so often that many people who could benefit from lip injections are terrified to get them in fear of having an unnatural-looking, protruding pout as a result. With what we now know about how much filler is needed for slight – moderate enhancement of the lips, it is clear that the duck lip look would take several syringes to achieve, and be rather costly.

You should not have to worry that you will have this type of result if you are only purchasing 1cc – 2cc of filler, and your injector should be willing to inject your filler in more than one appointment so that you can be sure you are getting the exact outcome you are looking for. Lips tend to swell a lot after being injected with dermal fillers, especially on the 2nd day after receiving them. Do not worry because the swelling will continue to go down.

The majority of first-time lip injection patients start out worried about getting too much put in, and then later end up wishing that some of the swelling would return after it goes down. This is why follow up appointments and knowing how much filler to use are so important. More can always be added at that time if that is what you and your physician decide.

Sausage Lips
A step “up” from the dreaded Duck Lips is the even more extreme Sausage Lips. This happens when the lips have clearly been overfilled to the point of losing their lip-like shape altogether, making them appear to look like two sausages placed on top of one another instead. Often times in these cases the filler will even go beyond the lip border, causing the area around the lips to swell out as well.

Needless to say, this is not a look that most people would like to have, however being overfilled to this extent is almost always done by request of the patient due to the high cost of needing several syringes to achieve it, as well as the fact that many physicians would not even agree to the treatment amount.

What if I Feel I Have Received Too Much Filler?!?!

If you do end up unsatisfied with your results, the good news is that the majority of dermal fillers that are used today are made of hyaluronic acid or sugar molecules. Hyaluronic acid can be dissolved with injections of a substance called hyaluronidase. Also, if you do not want to dissolve your fillers, remember that they are not permanent will be reabsorbed by your body over time. Most fillers last anywhere from 6 months to 2 years.

One of the most important things you can do before selecting an injector for your dermal fillers is to familiarize yourself with his or her style. Consider scheduling a consultation prior to your treatment. While many people fear having too much injected, it can be just as disappointing to spend money on filler treatment and be undertreated. Be sure to communicate exactly what you would like to see from your results, so they can give you an accurate idea of how much filer you will need.

The Most Important Habits For Weight Loss

Write down specific, measurable results for yourself that you are going to achieve, and read them every morning and evening.

Aim for a maximum of 2 lbs weight loss per week, and, if possible, focus on reducing body fat percentage. One of the best habits for weight loss to reward yourself when hitting certain goal. For example: My goal is to lose 15lbs and achieve body fat of 24%. I will lose 1.5 lbs per week, so I should achieve my goal in 10 weeks.

To do so, I will reduce my calorie intake to 1740 calories per day and increase my protein intake to 120 grams. I will commit to 3 strength training sessions (45 minutes) and 2 HIIT sessions (20 minutes) per week. I will walk my dog daily for 30 minutes. When I achieve my goal, I will reward myself by taking a trip to Oahu with my friends.

Declutter your kitchen.

Go through your refrigerator, pantry, and cabinets and get rid of all the foods that you may find tempting and that are not in line with your new lifestyle.

This is one of the most crucial habits for weight loss, especially if you are an emotional eater, crave junk food at the evening or find yourself stressed. Ensure that your healthiest items are the ones easiest to see.

If you have children or a spouse who eat differently, set up a space for their snacks away from your daily reach – for example place chips, sweets and other processed food in a bin that is stored in the garage.

Hopefully this will help them reach for a healthy food more often as well.

Track your food intake and exercise, and compare to your calorie, macronutrient and activity goals daily.

One of the habits for weight loss you should do is to have a plan and track it to stay focused and to better understand your body. You might also find that you need to tweak your goals as you progress.

We recommend using simple apps such as MyFitnessPal, CalorieKing or LoseIt. Tracking your weight loss progress can be tedious, but you will learn so much by doing this and will benefit from this knowledge in the future as well.

Plan ahead for success and meal prep.

Designate a food prep day to make sure that you have your meals and snacks ready. This is one of the best habits for weight loss you should follow.

We find that losing weight and getting in shape is not so much about having an iron will, but really about being prepared at all times, making this lifestyle a priority, and following these habits for weight loss.

Instead of wondering several times a day what you should be eating, spending time looking for the right restaurant or shopping for groceries daily, designate one to two days a week for meal prep. The best way to start is to search for meal prep guides on Google, or visit Pinterest and/or Instagram for great ideas.

Set up your meals for at least 3 days in advance, and carry a food bag with you to avoid the pitfalls of being hungry while on the go. We promise you will be able to stick to your goals more easily if you do not skip this step.

Always carry your healthy meals and snacks with you wherever you go.

You do not have to cook all your meals or snacks. Opt for fresh cut up veggies such as bell peppers, celery, carrots etc., and grab little snack-size packets of almonds or peanuts.

A small serving of hummus or guacamole makes a wonderful snack as well, and little tuna and salmon packets will keep your lean protein intake up without missing a beat. These make great habits for weight loss to follow.

Eat your first meal within an hour of waking, and then eat every three hours.

Another one of the important habits for weight loss is to never be too hungry or too full. We find that once you are starving, it is too late to try and stick to your goals, and it is near impossible to maintain a high enough protein intake without a 5-6 meal daily routine.

Consider using a whey protein shake for one to two of these meals if you find eating that many times per day is not practical for you. All your meals should include lean protein to help you stay satiated, and to protect your bones and muscle mass while dropping unwanted fat.

Consistency is key.

If you have a day where you slip up… don’t give up. Start fresh – with your next meal, or start these habits for weight loss the next day. Expect weight loss plateaus, setbacks and disappointments… keep in mind that your hormones, sleep, stress and more can be in play as well.

Healthy weight loss is never linear, it might be very easy to drop few pounds initially and then get stuck.

Keep working hard to bust through temporary plateaus, and continue to review all your efforts as it might be a time to adjust your nutritional intake or activity levels.

Reduce stress and get as much uninterrupted sleep as possible.

Aim for 7.5 hours of sleep per night, there is no badge of honor for missing out on sleep, and not getting enough sleep can contribute to weight gain and inability to lose weight.

The combination of low leptin and high ghrelin caused by a lack of sleep is likely to increase your appetite. In other words, short sleep might stimulate appetite, which increases weight.

Also, we find it more difficult to stay motivated and follow these habits for weight loss when we are tired.

Eat real food.

Your food should be mostly one ingredient, not packaged or processed. As you buy healthy wholesome foods, there is no longer a need to look for marketing messaging such as “low-carb” or “all-natural.”

One of the habits for weight loss to follow is to choose a low-carb diet, but do not buy “low carb” products. Opt for organic foods whenever possible, but keep in mind that just because something is organic, does not mean that it is healthy or conducive to reaching your weight loss goals.

Read all labels carefully and don’t trust any info on the front of a package as it is not monitored by FDA in the same way as the Nutritional Facts section.

Minimize/Eliminate sugar of any form from your diet.

We often find that our patients are eating the right amount of calories for weight loss, but are not losing weight. The culprit is usually sugar and not following these recommended habits for weight loss.

When you eat sugar, your body automatically goes into overdrive producing insulin, pulling glucose into your cells and prompting your body to hold on to fat for future use. Of course this causes weight gain, or stops your fat loss efforts.

Cutting sugar out of your diet is absolutely essential if you are looking to achieve great results. You might find that you are addicted to sugar and might have to fight through migraines, lack of energy, moodiness and other symptoms for several days to get rid of the addicting effects of sugar.

Get most of your carbs from vegetables – 75% of your plate!

Eat an unlimited amount of these veggies: green beans, broccoli, spinach, lettuce, kale, onions, asparagus, cauliflower, kale, cucumbers, and sprouts.

Green veggies help release stored fat and broccoli is great for hormone regulation.

When opting for other carbohydrate sources, look for ones which are minimally processed and do not contain added sugar such as beans, brown rice, sweet potato and oatmeal.

More important habits for weight loss are to stay away from bread, pasta, white rice, chips, crackers and fries as much as possible. Keep it simple!

Use the volume of food to your advantage.

By eating a large volume of low-density foods, you can satisfy your cravings while losing weight. The goal is to never be hungry while keeping your calories down and staying healthy.

Focus on eating foods that contain more water, such as fruits and vegetables. A great example of this would be a dinner dish of a huge salad bowl or spinach and arugula greens with fresh grilled chicken and tomatoes.

You are much less likely to reach for a snack if you are full and satisfied.

Hyaluronic Acid

The majority of dermal filler products on the market today are made of a substance called hyaluronic acid (HA). The hyaluronic acid that is used in these fillers is identical to the naturally occurring hyaluronic acid in our bodies. Radiesse is one of the commonly used fillers that is not made of hyaluronic acid and instead is made from Calcium Hydroxylapatite (CaHA). It was first used in dentistry and reconstructive surgery with a long safety record, and side effects are rare.

Radiesse is one of the best types of dermal fillers used for deeper lines, including the folds that run from the lower nose toward the corners of the mouth. With Radiesse, hyaluronidase cannot be used, but the product can be removed by extrusion, or broken up by injecting saline. Both products will be broken down and reabsorbed over time.
