RevitalizeMaui MD (dba RevitalizeMaui) (“the Clinic,” “we,” “us,” or “our”) provides physician-directed medical aesthetics and longevity services. By purchasing services or products at our clinic or through our Clover-hosted payment systems, you agree to the following terms.
1. Payment and Transaction Terms
By providing your payment information, you authorize RevitalizeMaui to charge your credit or debit card for services rendered and products purchased.
All payments are processed securely through Clover and Dharma Merchant Services using PCI-DSS compliant systems. RevitalizeMaui does not store full credit card numbers on local systems.
2. Refund and Cancellation Policy
Medical Services: Due to the nature of medical aesthetic and longevity treatments, services are considered rendered at the time of treatment and are non-refundable.
Prepaid Treatments: Unused portions of prepaid treatment packages may be eligible for clinic credit, less the value of services already performed.
Product Sales: Unopened clinical-grade skincare products may be returned within 14 days of purchase for clinic credit only.
Cancellations: A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. Late cancellations or missed appointments may be subject to a cancellation fee charged to the card on file.
Billing Concerns: Any billing concerns must be directed to the clinic for resolution prior to initiating a charge dispute.
3. Privacy Policy
We collect your name, phone number, and email address to provide receipts, appointment communication, and clinical follow-up.
We do not sell or share your personal information with third parties for marketing purposes. Information is shared only with payment processors, including Clover, as required to complete transactions.
Protected Health Information (PHI) is handled in accordance with HIPAA privacy standards.
4. Medical Disclaimer
All services provided by RevitalizeMaui are elective medical treatments. Outcomes vary by individual. Information provided on our website or in clinic materials is for general educational purposes and does not replace a formal medical consultation.
5. Governing Law and Contact Information
These terms are governed by the laws of the State of Hawaii. Any disputes shall be resolved in Maui County, Hawaii.
