Your $50 DEPOSIT will hold your appointment time, and be applied to the cost of your consultation when you come in. Initial Consultations are $249, and follow up appointments are $120.

Keep an eye out for an email coming from PDF Filler that will direct you to fill out our Health Questionnaire. There are required fields on the first and last pages, and then just fill out whatever else pertains to you. You will need to use a computer, not a phone or tablet, to complete the questionnaire. We request that you submit it back to us at least 24hrs prior to your appointment so that Dr. Okwuje can review it before you come in.

Please let us know a minimum of 24hrs in advance if you need to make any changes to your appointment time. Failure to do so will result in forfeiting your $50 deposit. This courtesy enables us to maintain a higher availability of our time for you as well as others. By scheduling an appointment, you are agreeing to our cancellation policy. Late arrivals may result in a shortened appointment.
